REGISTER: This means I am officially registering for classes by filling out this form, and that I WILL ATTEND classes for Spring II, 2014. I understand that if I register for classes, then I am expected to attend. I understand that if I decide not to attend classes, then I am expected to come to 1673 Patterson Office Tower to make sure someone knows of your decision. If you use this form to register and then do not want to attend you will get a refund based on our refund policy which can be found here: https://esl.as.uky.edu/refund-policy-iep.
PAYMENT: Students will receive a bill at their UK email. You must pay this by the due date on your bill. If it is late, you will have to pay an added fee of 1.25%. You have to pay in one of the following ways: (1) Check from a US bank, or Money Order, at Funkhouser Building, Student Billing Office Room 18 (Building #54 on a UK map). (2) Online at Link Blue/ myUK by credit card. (Follow this link to your myUK log-in: https://myuk.uky.edu/irj/portal. Once you are logged-in, go to the "Student Services" tab, and click on "Financials". You will see "Account Services" on the left-hand side of the screen and once you choose that option, you may go to "Web Payment Services".)
Government Sponsored Student DEADLINE: Government sponsored students have until March 16, 2014 to turn in an updated Financial Guarantee. If we do not have one by this date, you will not be registered for class and will not receive a schedule. Please email your Financial Guarantee to Ms. Natalie at email@example.com, or bring a copy to her at the CESL Main Office, 1673 Patterson Office Tower.
Self-Funded Student DEADLINE: Non-sponsored students have until Friday, March 16, 2014 to pay their bill in full to the Student billing office, room 18 in the Funkhouser Building. If our office does not receive a receipt of payment by this date, you will not be registered for class and will not receive a schedule. Please email a copy of your payment receipt to Ms. Natalie at firstname.lastname@example.org or bring a copy to her at the CESL Main Office, 1673 Patterson Office Tower.
ATTENDANCE: Students are expected to attend all classes they have registered for, unless specifically excused by the instructors. In case of illness, students must contact their instructors. Only legitimate absences, which have been reported, will be excused. All other absences are considered unexcused. 20% of class refers to how many times a class meet in a session. The following numbers may change if your class meets less often. After four unexcused classes, you will be required to meet with your Academic Advisor to discuss consequences and possible solutions. After seven unexcused classes, you will be required to meet with your Academic Advisor and possibly the chief immigration officer. Your immigration record could be canceled.
HEALTH INSURANCE: Please note the University of Kentucky requires that all international and ESL students with a J1, J2, or F1 Visa status have health insurance coverage. If you will have other insurance coverage and do not wish to have the student health insurance plan, you may apply to waive (cancel) the student insurance by submitting a waiver application. The DEADLINE to apply for the waiver for Spring 2014 was February 22, 2014.
